Prerequisites

  • Suitable premises and equipment.
  • Registered health professionals on staff.

Required Documents

  • Certificate of Incorporation and CIPA company extract.
  • Certified copies of Omang / Passports for directors and shareholders.
  • Proof of physical business premises (lease or title deed).
  • Tax clearance certificate from BURS.
  • Business plan and financial projections (3–5 years).
  • CVs and fit-and-proper declarations for key management.
  • Audited financials / proof of capital.
